Forms and Paperwork

The forms below may be downloaded to your computer, printed, and used as needed.  They are available as web links, in Microsoft Rich Text Format (.rtf), Microsoft Excel files (.xls) and/or Adobe Acrobat (,pdf) format.  The .rtf files should be readable by most word processing programs.  (For the .rtf and .xls files, try right clicking on the file name, selecting “Save Target As” or “Save Link As”, save the file to your hard drive, and then open it from your computer.)

Some forms are only in Acrobat format because the original document could not be easily converted into the .rtf format.  If you want or need the Acrobat version, you need to have the free Adobe Acrobat reader on your computer (which is some handy software to have anyway).  Use the link below to download the free software from Adobe, and then use it to view the forms you need.  Free download of Adobe Acrobat
